The Health Questionaire is a form that is used in case of an emergency; it may be filled out answering most questions that emergency response or emergency room personnel may ask you.  With this form we can get the information we need quickly and the information that is usually overlooked in a hurry.  Once filled out keep the form with you or at home in a place that you or a family member can access it.
